A 48-year-old patient presents to the hospital with syncope and hypotension.

Note the S wave in lead I and the Q wave in lead III.

***** ANSWER *****

Acute pulmonary embolism: Sinus tachycardia, the most common ECG abnormality, and the classic S1Q3T3 pattern are observed. Evidence of right heart strain, a right bundle branch block (RBBB) pattern, and axis deviation is also present.
